Client Assessment forms
For the purposes of meeting Insurance Company criteria as well as for your health & safety, you may be required to fill in a Physical Activity Readiness Questionnaire before joining a yoga class with Aina Maj.
Information submitted either on paper or digitally is kept securely for the time determined by the Insurance Company and then destroyed/deleted.
Nutritional Assessement forms are to be filled in in other for any nutritional recommendations to be made. These are kept on file for future reference.
All information gathered by Aina Maj either on paper or digital forms is treated as confidential and will not be shared with a third party. It is kept securely and only viewed by Aina Maj.
If you wish to alter any of your data, review how it is used or wish to request its deletion or return to you, please contact Aina Maj via email@example.com.
You may, of course, decline to submit personally identifiable information, in which case Aina Maj may not be able to provide certain services to you.

I do not use your phone number to contact you unless
1. you contact me first
2. you are a private student or client
3. in the case of an emergency schedule change
